A youth custodial officer working on the night Cleveland Dodd was found unresponsive in his cell in Perth, has described the teenager's death as a tragedy that could have been avoided. An inquest into the 16-year-old's death has heard guards working at Unit 18 within Casuarina prison sometimes watched movies while working the night shift to help them stay awake.
